At Baylor, we use the title care coordinators instead of case managers because your role is caring for patients rather than managing cases. Our care coordinators do far more than communicate with insurance companies. They improve patient outcomes by overseeing the continuum of patient care and providing a vital link between patients, payers and staff.
We have also implemented an innovative staffing model for care coordinators in some of our hospitals. The duties of the care coordinator have been targeted to empower them to focus on the patient care continuum.
Several departments help support care coordination throughout the Baylor system:
Regular communications from support departments keep care coordinators well informed about issues affecting their duties and what steps Baylor is taking to solve them.
There's a strong collaborative relationship between care coordination and other departments, which helps create a positive work environment for care coordinators. Care coordination and social work enjoy an especially productive relationship at Baylor.
Our nurses are empowered to make decisions that improve patient care. We are committed to a shared leadership model that encourages all staff nurses to be actively involved in setting the clinical policies and procedures for their unit.
At Baylor, you can work in your preferred care setting or experience a variety of settings. Our standardized best practices for care coordinators make it easy to transfer to another hospital or unit. From critical care to long-term acute care to medical/surgical and beyond, you'll find your ideal work environment at Baylor.
