Your team may consist of a primary care physician, consulting physicians, physiatrist, registered nurses, enterostomal nurses, certified chemotherapy nurses, social workers, respiratory therapists, physical therapists and occupational therapists, speech-language pathologists, therapeutic recreation specialists, clinical dieticians, discharge planners and chaplains if appropriate.
To determine if you are an appropriate candidate for our hospital, a medical assessment is performed by a nurse evaluator, either on site or via telephone with your physician. The evaluator will then confer with physicians, discharge planners, insurance case managers and family members to arrange a timely transfer if appropriate.
