1. How do I obtain a copy of a birth certificate for someone born at Baylor Medical Center at Carrollton?
Birth Certificates are completed by the hospital and sent to Texas Vital Statistics within five days of birth. Please contact the Denton County Clerk's Office directly at Metro 940.349.2010 or 972.434.8820 (Birth & Death Certificates).

2. How do I request a Social Security number for my newborn?
Parents may request a Social Security number for their newborn at the time of birth. The Social Security Administration will assign your newborn a number and mail the card directly to you. For additional information regarding Social Security cards, please visit www.SocialSecurity.gov or call 1.800.772.1213

 

3. How can I get my medical records from your facility?
Please call Medical Records at 972.394.2492 or via the Release of Information Request line at 972.394.2272. You will need to complete and sign an authorization for release of medical records/patient information and return it to the Medical Records department before they will release your records. 

 

4. Who should I speak with about my hospital bill?
Please contact our Regional Business Office directly at 214.820.3151
or toll free at 1.800.725.0024 for all hospital billing inquiries. They are located offsite and do not have an office within the hospital.

*Please note that certain physician groups, such as radiology, pathology and emergency physicians are not part of the hospital and perform their own billing.

5. I'll be giving birth at your facility. How many people may I have in the room with me?
Two is the maximum number of people who may be with you at one time during the delivery.

 

6. My friend or family member recently had a baby. Can I see their baby's photo online? Click here

 

7. I can't be there for my family member's surgery. Can I get a message to them?
Yes, please click here to try our E-mail a Patient function. Your message will be delivered usually within the same day or next business day, although on some occasions delivery may not be possible if the patient has been discharged before receipt of e-mail.

 

8. I'd like to take a CPR class. Does your hospital offer these to the public?
Unfortunately we only provide CPR training to our staff. Your Dallas American Heart Association provides certification classes. Contact them at 1.877.242.4277 or 1.877.AHA.4CPR or by clicking here to visit their website CPR page to find a class near you.

 

9. What is the closest train station or bus stop to Baylor Carrollton?
The closest DART bus stop is on Route 331, which stops right near the hospital. This is in quadrant A of the transit map. For more information, please visit the DART website at www.dart.org or their maps page at http://www.dart.org/maps.asp.

 

10. What are your policies regarding visitors for patients? Standard visiting hours are 9am - 9pm. Please note that the main doors lock at 9pm for security reasons, and all admittance to the facility after 9pm is via the Emergency Department on the northwest side of the hospital. 11. When is your cafeteria open for hospital visitors?
Baylor Carrollton's Cafe is located on the first floor and open to the public Monday through Friday from 7:00 am to 6:00 pm and on the weekends and most holidays from 7:00 am to 1:30 pm. The Cafe offers a wide selection of made-from-scratch recipes daily. Grab-and-go selections include pre-made salads, sandwiches and wraps, as well as desserts. The saute station offers a daily made-to-order selection. Homemade soups, a build-your-own salad bar and a beverage station are also available. Coffee service is available in the dining area 24 hours a day.

 

Vending machines for snacks and beverages are also located on the first floor near Patient Registration in the front lobby and near the Emergency Department waiting area. Additional vending machines are available on the patient units and are available 24 hours a day.
