Pain Guide

Regularly ask you about your pain, including its severity, location and duration.
Discuss with you your treatment options and create a pain management plan.
Regularly ask you if you pain medication is working.

Tell us whenever you have pain. Describe it in detail and use the pain scale to report intensity.
Take pain medication when the pain first starts.
Report any side effects you think you are having.
Ask us any questions you have about your pain medication.

Please be as detailed as possible about the pain as we ask you the following questions:

How does your pain feel? Possible answers include aching, stabbing, tender, numb, dull, crampy, throbbing, gnawing, burning, penetrating, miserable, radiating, deep, shooting, sharp, exhausting, unbearable, nagging, squeezing, pressure.
Where is your pain?
Is the pain always there or does it come and go?
What makes the pain worse and what makes it better?
How does the pain affect your sleep, appetite, energy, activity, relationships or mood?
Are you experiencing other symptoms with your pain, such as nausea, constipation, vomiting, itching, sleepiness or difficulty breathing?
How would you rate your pain using the following scale?

0 No Pain

1 - 2 Mild Pain

3 - 4 Moderate Pain

5 - 6 Severe Pain

7 - 8 Very Severe Pain

9 - 10 Worst Possible Pain
