Navigating Your Cancer Journey

Patient Navigation Program - Call 214.820.3535

Because the diseases of cancer are complex, Baylor Charles A. Sammons Cancer Center's Patient Navigation Program offers assistance to cancer patients and their families in navigating through the system for cancer care.

Following an initial cancer diagnosis, the different types of physicians you may see and the many decision you will make are often overwhelming.  That's why every family who walks through our doors has access to a patient navigator.

The patient navigator understands your needs and provides the ability for you to focus on healing.

A patient navigator is a registered nurse (RN) who is available to you and your family as an advocate, guide and moral supporter that understands your needs and will see you through your recovery.

Your patient navigator will:

  • Help you gather all your medical records
  • Coordinate referrals to cancer specialists and support team that has expertise in diagnosing and treating cancer
  • Assist with accessing specialists and scheduling appointments
  • Coordinate access to services such as education, support and identification of other resources needed
  • Answer general questions about cancer
  • Helping you and your caregiver understand your personalized plan of care
  • keep track of your treatment progress
  • Provide information to make your visits easier, such as directions, parking, transportation and hotels.
  • Provide information on cancer research trials.
