Nationally, only 38 percent of congestive heart failure (CHF) patients have a follow-up visit with a health care professional within seven days of discharge, even though patients who have a follow-up within seven days have a 15 percent lower 30-day readmission rate.*
That is why all CHF patients at Baylor Garland are automatically referred to the Health Management Center at Baylor Garland. The Center, which is part of the Baylor Garland Heart and Vascular Center’s Heart Failure Program, offers education and support to patients with CHF.
Open weekdays from 8 a.m. to 6 p.m., the Health Management Center educates patients about medications, smoking cessation, low sodium diets, and signs and symptoms of CHF.
Additionally, the Center makes referrals to primary care physicians or other qualified health service providers. If a physician or other qualified provider cannot schedule a patient for a visit within five days of discharge, the Heart Failure Program team will meet with the patient weekly until an appointment can be made.
