TEAM Diabetes Self Management Training Program

Registration for the TEAM Diabetes Self Management Training Program is simple:

Step 1: Your doctor completes and faxes the Diabetes Management Certificate of Medical Necessity (Referral Form) to the program administrator at (817) 922-1951.

Step 2: Once a referral is received, you will be contacted to gather additional information, determine which class schedule works best and to arrange a personal consultation with the diabetes educator.

This educational program is covered by many health insurance plans. Both insured and self-pay diabetes patients are welcome. Baylor All Saints Medical Center provides insurance verification prior to your appointment. It is advised to contact your insurance provider to confirm coverage details.
