Your First Appointment

You can download a medical profile to fill out and bring to your first appointment. Bringing this profile completed to your office visit will expedite your check-in. We also need a summary of your medications.

At your first appointment, please wear comfortable clothes you can easily move in and, if your appointment includes aquatic therapy, please wear or bring a comfortable, modest bathing suit. If desired, you may wear non-cotton based shorts and t-shirts in the pool as well.
