Before Your Appointment

When you arrive at The Baylor Wound and Hyperbaric Medicine Center for your first appointment you will be asked to complete some paperwork. You can complete this paperwork ahead of time by filling out these forms. Please bring them along with your insurance card for your scheduled appointment.
