Scheduling a Personal Edge Appointment

To begin your Personal Edge program, choose the package that's right for you or simply call (214) 363-2745 for an appointment.

We'll offer an appointment time that fits your busy schedule and offer you the ability to complete the required paperwork on-line in advance of your appointment.

A 10% discount is extended to spouses. Please provide this information to the Wellness Coordinator when scheduling the appointment.

We will be happy to make hotel reservations, ground transportation to and from the Personal Edge office, and round-trip airport limousine service for out-of-town guests. For your convenience, charges for these services can be added to your Personal Edge assessment bill.

If you need a wheelchair accessible suite, have special dietary needs, or if there are other requests we can help fulfill, please let us know.

Cancellations
If you need to cancel your appointment, please do so at least 48 hours in advance to avoid a cancellation fee of $350.
