TEAM Diabetes Self-Management Training Program for Type 1 Diabetes
Comprehensive assessment and review of self-care behaviors. A one-on-one education program designed to meet individual education needs.
TEAM Gestational Diabetes Education Program
Designed specifically for pregnant women whose pregnancy has been complicated with diabetes. The program consists of intensive, individual meal planning, blood glucose and ketone testing instructions, exercise plans as recommended by the physician and insulin therapy when required.
TEAM Insulin Pump Program
Comprehensive assessment, education and support for individuals choosing to use an insulin pump.
Medical Nutrition Therapy
Designed to meet the educational needs of the patient diagnosed with pre diabetes (Impaired Glucose Tolerance). Comprehensive assessment and meal planning with a registered dietitian. However, we see individualized meal plans as a need for everyone with diabetes even if they are unable to attend TEAM; as well as, a recurrent need as a persons diabetes treatment plan changes or other major life-style changes occur.
Continuous Glucose Monitoring System (CGMS)
The CGMS system is designed to monitor blood glucose levels. A flexible plastic sensor inserted under the skin measures and records an average blood glucose value every five minutes for up to 72 hours. The information from the sensor is then downloaded to a computer and detailed graphic reports can be printed. A physician will review the data with your diabetes educator and make recommendations for changes in your diabetes plan, if necessary.
TEAM Diabetes Types 1 and 2 for Children
Comprehensive family approach to the assessment and education of both established and newly diagnosed children and adolescents with Type 1 or Type 2 diabetes.
