Newborn Screening Packets

Supplemental Newborn Screening is available to you at $35.00 per sample. Please note you may incur an additional fee from your provider or lab draw station for specimen collection.

We require that you include the $35.00 payment at the time you submit the screening card. Checks should be made payable to: Baylor Institute of Metabolic Disease

The following information is included in every Newborn Screening packet:
  • A letter for your physician/caregiver explaining the screening program
  • Specimen collection information
  • Specimen collection instructions
  • Sample collection card- Please remember that this card must be used to obtain the sample for screening.

The optimal time to collect the sample is while your baby is still in the hospital and at least 24 hours after delivery.

You will want to discuss this screening with your physician or caregiver prior to your baby's birth. Your physician or caregiver will need to make arrangements with the hospital staff to obtain the blood sample which may be performed from the same heel stick used to collect the state mandated screening. If you do not have the specimen collected while your baby is in the hospital, you can ask your baby's pediatrician to collect this sample for you during your first office visit.

The following items must be included when you or your physician submits the sample for the screening:
  1. Signed Informed Consent, which includes the signature of the parent or legal representative of the baby, and a witness signature, is required- the resulting informed consent is located on a label affixed to the glassine envelope. Screening or result reporting may be delayed if the card or informed consent is not complete.
  2. A doctor's signature on the screening card is required.
  3. Payment must accompany the sample
  4. The mailing address of the designated healthcare provider (for result reporting).
It is very important that you do not handle the screening card as this could cause contamination that could potentially alter or interfere with the screening results. The card must be left in the protective envelope until the healthcare professional is ready to obtain the sample.

Results are reported to the designated healthcare provider.
Laboratory staff are not permitted to provide patient results to anyone other than the designated healthcare provider or ordering physician without a signed, written authorization from the parent or legal representative.

Our lab professionals are available to discuss supplemental newborn screening results with the physician so treatment can begin and referrals can be made if necessary.
